I was included in boat trips from birth, and my dad did a few things to make sure I was safe, and comfortable with the water. First and foremost is a good life vest with auto-head up attitude. The other thing is that you can't take your eyes off this child AT ALL. I managed to fall off a 16' hobie cat with both my parents sitting right next to me when I was 4. And make sure the kids aren't scared of the water, by taking them swimming often and teaching them to swim as soon as possible.

As far as the securing of the child, your lap is the best bet, though I know a montauk doesn't exactly give a secure seating place for the adult.
